New Patient Forms

Has it been a while since your last visit to the dentist? Are you new to the area and looking for a local dentist in Ames or a dentist in central Iowa? Are you a student at Iowa State looking for local dental services while at school? No matter your reasoning, we’re excited to welcome you to Ames Family Dental!

During your first visit to our dental office, you’ll be welcomed by our team. They will offer you a tour of the space and answer any questions you have. We ask that you fill out our online new patient forms before your first appointment.

How much does a certain procedure cost?

We perform over 200 different procedures in our office. To give you an accurate estimate of a certain procedure’s cost, we will need to schedule an appointment. This will allow us to see what products or services are best for your oral health.

What types of dental insurance do you accept?

Dr. Patten is an in-network provider for Delta Dental Premier and Wellmark Blue Dental. However, we "accept" payment from most insurance carriers- they can send us a check directly and we can "accept" it as payment toward the treatment provided. This is different than being "in-network" with a specific insurance plan.

Are you accepting new patients?

We are always excited to welcome new patients to our dental practice! Please fill out our new patient forms before your first appointment.

Do you provide pediatric dental services? How young of children will you see?

Yes! We would love to be your pediatric dentist of choice. We will see patients as early as one year of age for a consultation and what we call a ‘Happy Visit’. We usually begin cleanings around the age of three.

How soon can I get in for an appointment?

We will make every effort to accommodate your schedule. We have new patient cleaning appointments each week and dental emergency times reserved each day. Dental emergencies are seen the same day- we know that dental problems and dental pain can’t wait.

How long will my appointment take?

It depends on what your appointment is for. Most routine dental check-ups are about an hour in length. Some appointments are only 20-30 minutes. Others, such as root canal treatment or diagnosing an abscess are longer.

Our address is 2625 South University Blvd in Ames, Iowa. We are conveniently located two blocks south of HWY 30 on University Boulevard near the junction of Airport Road and Oakwood Road. Our phone number is 515-233-2898 if you need directions.
